From 4 to 10 February 2021, we carried out an audit of the handling of an incident involving a gas leak from a flow line at Gullfaks B on 5 March 2020.
Objective
The objective was to audit Equinor’s handling of the incident of 5 March 2020 involving a gas leak from a flow line, in order to learn lessons from the incident.
The audit also comprised an assessment of whether relevant factors for learning were included in the investigation report and in the measures followed up by the company after the incident.
Result
Three non-conformities were identified, relating to:
- Verification of performance requirements for operational and organisational barrier elements
- Description/specification of the effect of measures
- Sharing of information
No improvement points were identified.
What happens next?
The report contains a description of identified non-conformities, and we have asked Equinor to report on how these non-conformities will be addressed. The deadline for this is set at 10 May 2021.
