The audit was conducted between 20 November and 15 December 2017.

Objective

Our objective was to monitor Statoil's administration and management of materials handling, safe use, maintenance and procurement of lifting equipment, and work at height.

The audit of Åsgard A paid particular attention to technical factors, the maintenance and the condition of lifting equipment, as well as findings following enterprise-of-competence controls.

Verifications and compliance measurements in relation to regulations and standards have previously been carried out by both the PSA and Statoil at Åsgard A, and the facility's lifting equipment shows signs of age.

In 2010, the PSA conducted an audit in which a number of technical non-conformities were discovered that were not followed up.

Statoil itself has identified and documented similar conditions through compliance measurements, and annual enterprise-of-competence controls have also commented on this.

In the audit, we made a number of extensive observations at Åsgard A that show a pervasive lack of follow-up of non-conformities, inadequate follow-up of findings from enterprise-of-competence controls and deficient maintenance.

The observations are based on Statoil's presentations, documentation, our interviews with personnel and verifications on board Åsgard A.

Result

The audit identified several regulatory non-conformities. These concerned:

  • Follow-up of non-conformities for offshore cranes
  • Follow-up of findings from lifting-equipment controls
  • Maintenance of offshore cranes and lifting equipment
  • Deficiencies in scaffolding

We also identified an improvement point relating to work at height

We have asked Statoil to give an account by 20 April 2018 of how the non-conformities will be addressed and to provide an assessment of the improvement point we observed.
