Background for the audit
We conducted the audit on the basis of the provisional regulations of 19 December 2003 relating to safety and working environment for certain petroleum facilities on land and associated pipeline systems. The audit is also based on the many reported incidents involving use of lifting appliances.
Purpose of the audit
The purpose of the audit was to verify compliance with applicable regulations in the areas of lifting equipment and lifting operations, as well as to verify measures implemented by Norsk Hydro following undesirable incidents.
Result of the audit
Based on conversations with employees of Norsk Hydro and some of the contractors, as well as other technical observations made during the audit, it is our opinion that the management system used by Hydro to ensure that the contractors comply with regulatory requirements in the area of lifting operations and lifting equipment is deficient.
Other nonconformities are linked to factors such as deficient approval of lifting appliances, operation of mobile cranes with non-functional mandatory safety equipment, deficient user documentation and certificates of compliance/certificates for lifting appliances and lack of documentation of annual inspections of such equipment.
We have, however, noted that Hydro's organization includes two persons with specialist backgrounds who follow up the contractors' use of lifting equipment.
We have issued the company a notification of order as follows:
Pursuant to the provisional regulations relating to safety and working environment for certain petroleum facilities on land, Section 51, cf. Sections 10 and 11 relating to management systems, cf. Section 5, No. 1 of the HSE Regulations and Section 36 of the provisional regulations relating to procedures, cf. Section 4, second subsection relating to who is subject to the regulations, Norsk Hydro is ordered to:
Review the effectiveness of, and improve the management system that is used to ensure that the contractors comply with regulatory requirements within the area of lifting operations and lifting equipment, so that the requirements of the regulations are safeguarded, cf. Section 1.1.1 of the report.
The deadline for complying with the order is set at 1 December 2006. Please inform us when the order has been carried out.
We have asked Norsk Hydro to submit any comments it might have regarding the notification within two weeks after receipt of our letter.
