Audit of Talisman - logistics planning and implementation in the supply chain

The Petroleum Safety Authority Norway (PSA) conducted an audit of Talisman Energy Norge AS (Talisman) during the period 18-22 September 2006. The audit was aimed at how Talisman manages technical, operational and administrative aspects associated with logistics planning and implementation in the supply chain to and on Gyda.

Background for the audit

The audit was a verification activity in accordance with the PSA's primary project tasks for logistics and follow-up of lifting operations.

The audit targeted the risk associated with materials handling, including lifting operations, as well as the problem areas highlighted in the PSA's report "Causal connection analysis of unintentional offshore crane incidents" (link).

Purpose of the audit

GydaThe purpose of the audit was to verify that safety on Gyda (pictured) is ensured through Talisman's management of logistics planning and implementation.

Another objective was to verify that the technical condition of equipment is safeguarded through checks and inspection of lifting appliances and lifting gear, and that the follow-up of undesirable incidents involving lifting operations complies with relevant requirements.

Result of the audit

We identified nonconformities in relation to regulatory requirements during the audit. The cranes were modified, but no documentation could be presented to ensure that the modifications had been completed, and that the work was conducted in accordance with basic requirements. Technical faults and deficiencies were also identified on the cranes.

As regards materials handling, some challenges and improvement areas were observed during the audit. The training system could be improved.
