Audit of managment of activities and prevention of incidents on Heimdal

During the period 23 May - 22 June 2006, the Petroleum Safety Authority Norway (PSA) conducted an audit of Norsk Hydro Produksjon a.s' (Hydro's) management of activities and prevention of incidents on Heimdal.

Background for the audit

To follow up how the operators on the Norwegian Shelf use incidents as a learning experience, and how they prevent incidents through managing their own activities, is a priority commitment area for the PSA. This will be continued in our priorities vis-á-vis Hydro in 2006 where follow-up of incidents is a key issue.

Heimdal has experienced several serious incidents in 2005 and the beginning of 2006, and has therefore been chosen as the object of an audit in this respect.

Purpose of the audit

The purpose of the audit was to follow up how Hydro prevents incidents on Heimdal through management of selected activities.

The audit comprised the following issues:

  • Management of simultaneous activities.

  • Follow-up of and learning after incidents.

  • Requirements related to specialized skills and follow-up of this.

  • Initiatives to ensure understanding of risk.

  • Follow-up of the working environment.

  • Maintenance of safety-critical equipment.

Result of the audit

Through meetings and interviews we were informed that people were generally happy on Heimdal, and it was also confirmed that some useful improvement measures had been implemented in a number of areas in recent years.

We noted a high awareness of the importance of follow-up and learning after major incidents and good routines related to control of work permits. The integration of maintenance contractor Reinertsen seems to have been successful on Heimdal.

During the audit we identified two nonconformities related to management of maintenance and to the resource situation in the operations department, as well as three improvement issues related to implementation of the APOS management system, handling of incidents and management of expertise.

Beyond this, we have some comments related to other conditions.
