Background for the audit
The audit was a verification activity in accordance with the PSA's paramount project task for logistics and follow-up of lifting operations.
We focused on material handling risk, including lifting operations and the problem areas that appeared in the PSA's report "Causal connections with offshore crane incidents" (link).
Purpose of the audit
The purpose of the audit was to verify that safety is ensured through Maersk's management of logistics planning and implementation.
Furthermore, to verify that the technical condition is safeguarded through checks and maintenance of lifting appliances and lifting gear, and that the follow-up of undesirable incidents during lifting operations meet relevant requirements.
Result of the audit
We made observations of a technical, operational and organizational character.
We would particularly mention the following:
Mærsk Guardian was put into operation without Maersk having a satisfactory overview of the technical condition, status of maintenance and lifting equipment non-conformities on board. This was in spite of several of the non-conformities being pointed out by the PSA in the audit in January 2007 (link).
The training system has an improvement potential.
The lifting manual contained non-conformities in respect of the recommended standard, NORSOK-R003, and these had not been identified and formally processed as non-conformities.
